All balances not paid by your insurance company will be your responsibility.
If you do not provide an active insurance card, you will be asked for payment in full at the time of your visit. If your insurance company is non-participating in our practice, we will file a claim as a courtesy, but payment in full will be due at the time services are rendered.
Your responsibilities and our policies regarding payment are outlined on on our patient registration form, which you will be asked to fill out and sign before your first visit. It can be found online here.
